New Patient

The surgery welcomes new patients. A longer appointment may be required for doctor to take a detailed history and discuss the current problem.

A new patient information and consent form will need to be completed at the surgery. Please ring if you wish to make an appointment with us.

Returning Patients

It is important we have the correct patient information, when you present to reception, please have your Medicare card or drivers licence available and be ready to clarify details. If any of your details have changed, please let us know.

Fees

Please note that Dao Yuan Medical Group of clinics are all mixed billing. Individual doctors set their own fees, which are payable on the day. For any enquires, please call.

Payment methods

In order to keep our fees to a minimum we require payment at the time of consultation. For your convenience the Practice has EFTPOS facilities available, and in most cases, we can electronically process your claim immediately.

Access to Information

Personal health information about a patient will only be released in accordance with the relevant privacy laws and at the discretion of the patient’s usual general practitioner.  Fees Apply

Results

Practice policy is for patients to attend the surgery to discuss the results of all investigations (blood tests, x-rays etc) performed. Reception cannot give out results.

We find this is the most reliable and effective way to communicate the results to you and gives you an opportunity to ask questions of your doctor

Appointments

Appointments may be available on the day, and every endeavour will be made for you to see your doctor of choice however we cannot guarantee this.

If you need a long consultation, please inform the reception staff when booking your appointment.

Some examples of times when a long consultation may be required are for pap smears, immunisations, insurance consultations & medicals.

Please inform the reception staff if you require an appointment for more than one family member.

Telephone consultation can be used for quick scripts, to get results and for recurring referrals (not available for all doctors). Whilst we are happy to accommodate telephone consultations, please note that under Medicare Guidelines, you must present in person to the clinic at minimum once every 6 months.

We understand that life can get busy and sometimes plans change. If you are unable to attend your scheduled appointment, we kindly ask that you notify us in advance to avoid a cancellation fee.

Zero Tolerance

Under no circumstances will verbal and/or physical abuse towards doctors and/or staff be tolerated. Any person found doing so will be asked to leave the premises and the authorities will be called. Any abuse or threats will be reported to the local police station, and you will no longer be welcome in the clinic.

All doctors are independent. Please discuss any concerns you have with your doctor or email the manager who can follow up any problem on your behalf. If you wish to take a complaint further, you can ring the Health Complaints Commissioner on 1300 582 113.

Privacy Policy

All information collected by this practice is deemed to be private and confidential. The right of every patient is respected.

This practice complies with federal and Victorian privacy regulations including the Privacy Act 1988 and Privacy Amendment (Enhancing Privacy Protection) Act 2012 as well as complying with standards set out in the RACGP Handbook for the management of health information in general practice (3rd edition).

Under no circumstances are members of the practice team to discuss or in any way reveal patient conditions or documentation to unauthorised staff, colleagues, other patients, family or friends, whether at the practice or outside it, such as in the home or at social occasions. This includes patient’s accounts, referral letters or other clinical documentation.

Independent service provider and other team members are aware of confidentiality requirements for all patient encounters and recognise that significant breaches of confidentiality may provide grounds for disciplinary action or dismissal.

Every member of the team is aware of our Privacy Policy and has signed a privacy statement as part of their terms and conditions of employment or contract. This privacy statement continues to be binding even after the employment or contract has terminated.
